Certification
New Patients
Initial Certification

During this visit we conduct an assessment and ask you about your medical history. As part of the certification process, we will collaborate on your care and help you navigate through the process. Certifications are valid for 3 years and if you do not qualify, there will be no charge.
Renewal of Certification
$99
Re-certification every 3 years is required by the State of MN. You can schedule up to 90 days prior to your expiration date.
Existing Patients
Renewal of Certification
$49.50
We believe in rewarding loyalty. Returning certification patients receive 40% off their renewal visit. The discount will be applied at the time of your appointment.
